A MESSAGE FROM THE SCHOOL NURSE:

All incoming 12th-grade students need a second dose of meningococcal conjugate vaccine (MCV) on the first day of school, or risk exclusion from school as the provisional period for vaccination compliance is 5 days. Documentation that your child received this vaccination or a letter of exemption must be on file in the nurse’s office prior to the first day of school.  School vaccination information is supplied under the FORMS section below.

All students entering 11th grade need a physical exam by a health care provider, please provide a copy of your child’s physical exam to the school nurse as soon as possible.  A physical exam form is provided under the FORMS sections below.
